Duties & Responsibilities:
- Deliver timely, impactful support to clients, collaborating with the Lifestyle and Community Participation Manager.
- Lead skills-based, recreational, and social activities, ensuring client programs align with their needs.
- Liaise with clients and support networks to design tailored services, fostering community access.
- Monitor service delivery in compliance with the NDIS Quality and Safeguarding Framework.
- Support staff in identifying and documenting changing client needs while upholding privacy and dignity.
- Foster positive relationships with clients and families, preparing monthly reports for management.
- Collaborate with leaders and other units to plan and implement effective solutions.
- Serve as an escalation point, assisting senior staff with special projects.
- Identify and address training needs, conducting supervision catch-ups for team development.
- Create a positive environment for both employees and clients.
Skills & Experience:
- Person-centered approach with a focus on tailoring services to individual needs.
- Excellent communication skills with the ability to establish rapport with clients and their families.
- Passion for leadership, with experience in leading staff and maintaining high service standards.
- Demonstrated success in a similar role within community-based inclusion work service delivery for people with disabilities.
Special Conditions:
- NDIS Quality, Safety and You Certificate.
- Child Safe Environment certificate (if working with children under 18 years).
- Current NDIS Worker Screening.
- Current Working with Children Check (WWCC).
- Demonstrated success in a similar role.
- Experience in community-based inclusion work service delivery for people living with disabilities.
- Knowledge of NDIS legislation and funding requirements.
- Excellent understanding of WHS processes and standards.
