About the role
Have you worked within Aged Care with Community Care, then read on. Our client is seeking an experienced Community Care Coordinator to join their Community Development Team, for a temporary assignment working 1 - 3 days per week, commencing mid-January 2021. Reporting directly to the Senior Community Care Officer, you will be required to assist in the coordination of the Home & Community Care and the National Respite carers programs in the Murray Bridge community.
Duties
Develop, oversee, implement and regularly review Individuals care and transition plans
Complete and maintain all administrative requirements, including assessments, notes and reporting as required
Identify when a customers needs have changed, or risk are presented.
Conduct reviews and referrals for other services
All other related duties as directed
Skills and experience
Essential, tertiary qualifications in Community Aged Care or appropriate related Human Services disciplines
Experience in Community Aged care or appropriate related human services disciplines
Experience promoting Community Services
Experience in working with people with a disability or aged care related illnesses
Must have knowledge and understanding of current policies and programs relating to Community Care target population
Understanding of Local Government
Must be able to interact enthusiastically and empathically with a wide range of people
Be able to work with limited supervision and can work independently or within a team
Have strong time management skills
The ability to maintain a high level of professionalism
Be team orientated
Possess empathy, patience and understanding
Have strong verbal and written skills, with the ability to communicate effectively across all stake holders
Ability to access clients against defined criteria
Be flexible and adaptable
Valid driver's Licence
Current National Police Clearance, and relevant current DHS Screening Checks
